Important Information Regarding Health Coverage Tax Document: Form 1095-C
1095-C forms are now available electronically to all active, full-time employees via Employee Access. In accordance with the Paperwork Burden Reduction Act, BCSD will no longer automatically furnish paper 1095-C forms.
Individuals who wish to receive a paper copy may request one at no cost by:

The form will be mailed to the address on file within 30 days of receiving the request. Please verify your mailing address on file when requesting this form. Address changes can be made via Employee Access or by contacting Human Resources.
Please note that form 1095-C is not required to file a federal income tax return.
