Both parents and students must create an account and link them together.  This will be the only way athletic paperwork will be taken. 

Physicals must be dated after April 1, 2026 in order to be valid.  Please use all online forms when filling out information.

Only three items that should be uploaded.
1.  Doctor’s Physical Exam dated after April 1, 2026 with doctor signature
2. Medical History Form
3.  Birth Certificate
