This form must completed in entirety and all required documents uploaded before taking the $50 payment to the PSH front office. The payment can be in check or cash.
Documents you will need to complete this form are: valid driver's licence, proof of insurance, vehicle registration.
The completion of this form is not a guarantee of parking in the student parking lot.
PSH student parking can be revoked by administration without refund at any point during the year for violations of the student handbook.
The name and photo associated with your Google account will be recorded when you upload files and submit this form
