Registration Information

Age Requirements:

-      4K students:  4 years old on or before September 1, 2018

-      Kindergarten students:  5 years old on or before September 1, 2018

The following items are needed for registration:

Kindergarten – Fourth Grade:

-      Certified Birth Certificate

-      Two proofs of residency, which must be dated within the last 30 days

o    Examples:  electric bill, mortgage statement, lease agreement, land line phone bill, etc.

-      South Carolina Certificate of Immunization (must be on the DHEC form)

-      Social Security Card (optional)

-      Parent’s driver’s license

4K (CERDEP):

In addition to the above items, you will also need:

-      Medicaid Card (if applicable)

-      2017 Tax Return or W2
